Christiana Care Health System in Newark, Del., is leading the way in implementing age-friendly care by adopting evidence-based practices called the 4Ms Framework. The John A. Hartford Foundation and Institute for Healthcare Improvement in partnership with the AHA and Catholic Health Association of the United States have launched the Age-Friendly Health Systems initiative using the framework to improve care for older patients.
