Equity Transformation Partner Application
The AHA Equity Transformation Partner (ETP) program is available to highly reputable, qualified service providers in the diversity and health equity space.
The AHA’s internal team of experts, led by senior management of the Institute for Diversity and Health Equity (IFDHE), reviews and selects providers and services that meet our criteria, which includes the development of an exclusive offering specifically for AHA members. We are seeking organizations that:
- Have an established hospital–health system client base
- Can provide two case studies featuring recent work and three references from recent clients
- Demonstrate alignment to the Health Equity Roadmap framework
- Show commitment to continuous improvement of practice and sharing best practices with the field
Program Application: The application process has two parts - information collection on the form below and submission of case studies. To begin the evaluation process, please complete the application in full.
The ETP review committee will be in contact if questions arise or additional information is needed to complete the vetting process. The committee will provide final determination of application acceptance.
